COACHES/TEAM CONTACTS: 

When you create your roster, please select "CREATE A TEAM". Add your team name along with grade and gender. For the team captain, please put coach name. The roster must be completed before parents/guardians can complete the player waiver.

PARENTS/PLAYERS:  

Please select "JOIN TEAM" and select the correct team you belong to when filling out your waiver. (Ask your coach if you are unsure). Those filling out the waiver form should never have to select "CREATE A TEAM".

DO NOT COMPLETE THIS PART WITHOUT READING THE INSTRUCTIONS TO ENSURE YOUR "TEAM NAME" FORMAT IS CORRECT.

My child has permission to participate in Pacesetter events with the understanding that neither the facility host nor Pacesetter Sports will be held liable for any injury or illness incurred at the event. I also understand that any photos taken may be used for promotional use by Pacesetter. I also understand that the mailing and email address provided may be used by Pacesetter to send information to our family.
